There are some books e.g. The Lord of the Rings, Harry Potter, One Flew Over the Cuckoos Nest that are adapted really well but for every one of them there's a Flowers in the Attic, The Scarlet Letter and I Robot, so it really depends on the Adaption.

Very rarely. The one real exception is Lord of the Rings. Those movies were fantastic. The Harry Potter movies are okay, but nowhere near as good as the books.
